Online Registration - Child Pass

CHILD REGISTRATION FORM

This form can be used to request a child’s pass for Anime Fusion, for children age 5 and under, free of charge. These passes are used to track emergency contact information in case the child is separated from their party at the convention.

Any attendee with a Child pass must be chaperoned by a registered adult at all times while at the convention.
